• Your passport must be valid for at least 6 months, counting from the day you leave China.
• Your passport must contain at least one blank, completely clear “VISAS” page (amendments and endorsement pages do not count).
• Your passport must be signed and in good condition. The consulate staff may reject your passport if they find it to be in damaged condition, even if you don’t think that’s the case.
• If your passport shows that you were born in China, Taiwan, Macau, or Hong Kong, please make sure to follow the instructions for Additional Requirements in the tabs below.

If you have been issued a visa to China in the past, you must submit your original passport (even if expired or cancelled) showing your last or most recent previous visa to China. The Chinese consulate may disregard this requirement at their discretion, but it is a requirement and you must have it available for submission

How to work with your forms:
Download: Click the link above that corresponds to your trip to save (download) the PDF file to your computer.
Fill-out the forms: Open the PDF file and type-in your answers directly on the PDF form pages.
Print: When you are finished, print all your forms in black ink on white paper only, and sign them by hand. Make sure that all your signatures match the signature on your passport.

• ONE 2×2 passport size/type color photos on white background (no photocopies)
• Please write the traveler’s name in the back of the photo
• Glue one photo to the application form, and paperclip the other to the passport

• A simple, clear photocopy of your VALID driver’s license or state-issued ID

• A simple, clear photocopy of your student ID from USC

• Flight itinerary print-out
• USC Support Letter addressed to the consulate of Brazil

• Former Chinese citizens who obtained another citizenship must present one of the following:
  • Their most recent previous Chinese passport
  • Their most recent previous Chinese travel document used to enter in the USA
  • Their most recent Chinese visa issued to them
• Your original Chinese name must be written in Chinese characters in the appropriate field on the visa application form
• Proof of name change:  If the applicant’s name has changed since birth, please provide the original court order, naturalization certificate, or marriage certificate showing the official name change
