• Your passport must be valid for at least 6 months, counting from the day you leave Brazil.
• Your passport must contain at least one blank, completely clear “VISAS” page (Amendments and Endorsement pages do not count).
• Your passport must be signed and in good condition. The consulate staff may reject your passport if they find it to be in damaged condition, even if you don’t think that’s the case.
  If your passport shows that you were born in Brazil, please make sure to follow the instructions for additional requirements in the tabs below.

CLICK HERE to get started on your actual online application form. The link above will take you to a Brazilian government online visa application system. Forms cannot be hand-written offline. Change the language to English by using the pull-down choice on the top-right of the webpage. Click on VISA to start the application. You must answer your questions online and then print the final bar-coded one-page form when finished. For verification purposes, please also print each page as you progress on the online process:

When filling out your online visa application form: Be very thorough and careful when filling out your questions on the form. Enter all dates exactly as requested. Please enter all your names exactly as they actually appear in your passport. Do not omit, abbreviate, or add extra names or nicknames. Do not include titles and/or extra letters not contained in your passport. All added data that is not identical to your legal documents will cause your process to be rejected. Print the application page when you are finished in black ink on white paper only, and sign the form by hand. Make sure that your signature matches the signature on your passport.

  • A 2×2 passport size/type color photo on white background (no photocopies)

• A simple, clear photocopy of your VALID driver’s license or state-issued ID

• A simple, clear photocopy of your student ID from USC

• Flight itinerary print-out
• USC Support Letter addressed to the consulate of Brazil

• You must submit your original US immigration documents (submit all that apply to you): I-94, permanent resident (green) card, F-1, I-20, advance parole originals, etc.

• Former Brazilian citizens who obtained another citizenship must present one of the following:
  • Their most recent previous Brazilian passport
  • Their most recent previous Brazilian travel document used to enter in the USA
  • Their most recent Brazilian visa issued to them
• Proof of name change: If the applicant’s name has changed since birth, please provide the original court order, naturalization certificate, or marriage certificate showing the official name change
